It looks like this actually occurs whenever "dpkg-reconfigure network-manager" is run. It appears to take down all of USB. If I unplug/plug my USB devices after doing this, they fail, too.

Also, the machine won't reboot (it fails to shut down apache2), so it must be powered off by disconnecting the power.

network-manager guys: I cannot distill something from this bugreport, as I haven't changed anything in this regard. Could you please have a look at it?

this is probably a network-manager crash during upgrade, which is triggered either by bug 145683 or bug 85113

Can you please attach your /var/log/syslog that includes the time/date this incident occurred?

By the way, that log was taking during the lately network manager upgrade (to 0.6.5-0ubuntu15)

OK, this seems to be due to an interaction with the bcm43xx driver. If I do a "sudo modprobe -r bcm43xx" before upgrading or doing "sudo dpkg-reconfigure network-manager", then everything is OK.

I still can't get any useful info on what exactly is borking when bcm43xx interacts with a network manager restart, but that seems to be the issue.
