dm_map mod not loaded when booting with persistent=cryptsetup

Bug Description

When booting appending the persistent=cryptsetup cheatcode, complains about need to load dm_mod (device mapper) module
Fixable by adding

modprobe dm_mod

in a couple of places.
 Patch attached

This package has been removed from Ubuntu. It has been replaced by the live-boot package. If you still have this issue with the live-boot package in 12.04 or later, please reopen and reassign to that package.
