Add install instructions for websockets to README (and other cleanup)

Bug Description

OpenSRF version: master / 2.4 series

With the addition of bug 1268619 and support for websockets in OpenSRF, we should clean up and merge the separate README.websockets in with the main README file for OpenSRF.

The work I've begun so far involves a couple different commits:

1) Add the README.html generated output to the .gitignore
2) Minor doc styling tweaks to the existing README (bold "root" Linux account name)
3) Remove trailing space from the apache2.conf file for websockets
4) Add a new apache2.conf file for Apache 2.4 (necessary for Ubuntu Trusty and other Linux distros that use that version)
5) Add the new websockets instructions for OpenSRF README and remove README.websockets

Working branch to follow momentarily once I get the LP number for this new bug.

6) Include mention about the need to install an SSL certificate in /etc/apache2/ssl
