Affects me too after upgrading to 15.04.
When installing the packages, apt-get dist-upgrade terminated with an error processing mysql-server.
I repeatead dist-upgrade, it installed mysql-server and another sql-related package I don't remember flawlessly.
The 'report this' window popped up when logging in at the first reboot after that install of the mysql-server 5.6.25-0ubuntu0.15.04.1.
I have no idea what exactly went wrong and afaik did not use or change mysql at all.

affects me too after upgrading from Lubuntu 15.04 to 15.10.

Tried to reinstall the package because with "systemd-analyze" I get the answer: "boot not yet finished. Try later".
In fact, the boot finishes after 20 min.

I traced the problem back to mysql.service. Consequently, I tried to re - install mysql-server-5.6.0
