webstaff: final sprint2 merge

This is a omnibus for the final changes for sprint 2 of the browser client (cataloging):

- tweak display of fixed fields in MARC editor
- show index and hit count in the back to results button

  - Transfer Items to Previously Marked Volume
  - Cancel Transit
- better handling of duplicate or missing barcodes in
  volume/copy editor
- better handling of missing call number in volume/copy
  editor
- add ngToast for notifications and use it to display
  notifications during transfer actions in the holdings
  view

The pull request branch will also include the patch for bug 1539084.
