fast-user-switch applet crashed; panel offered to restart, but it crashed again

I saw exactly the same behavior. The applet is crashing when kdm is used instead of gdm. I don't have space on my hard drive to install the debug stuff so I cannot provid the file you asked but I'm pretty sure that you can reproduce it very easly (it's suffice to reconfigure gdm to kdm: sudo dpkg-reconfigure gdm), log on a gnome session and watch the applet crashing.
