confusing layout in Add Score System form

The "Submit" button should be at the bottom next to "Cancel" as it is with every other form. Since it isn't very clear that you aren't saving anything until you hit "Submit," I'd also say this is a case where it should say "Save" instead of "Submit."

"Update" should be more clear about what it is actually for -- "Add a lower score."

I'd also put the title field and score table inside a fieldset to make it more consistent with other forms, with "Save" and "Cancel" outside the fieldset.
