Binary package hint: rhythmbox
This seems to occur when playing a media file while the Rhythmbox window is open, and then closing the player using the (X) button. The sound applet continues to show the file playing, but does not actually play any audio, and does not change it's status from appropriate user input.

This happens to me after quitting rhythmbox even if I'm not playing any music files. Also, it doesn't disappear from the volume control after quitting.
