Don't ask for confirmation on actions

When users click on actions like "Submit for review" or "Publish" or "Unpublish" I don't think we should be asking for confirmation.

If anything, we can include an 'Undo' option.

I realise this was done because the actions moved from being buttons (in a post form), to links in a side-bar (and links must be idempotent), but we can still use html buttons in the sidebar and style them as links.
