Bug Description

Looking at the following example underlines the whole line:

__lorem ipsum__ __lorem ipsum__ foo __lorem ipsum__

Whereas the in the following example the "foo" is neither italic nor bold, which is correct.

''lorem ipsum'' ''lorem ipsum'' foo ''lorem ipsum''

'''lorem ipsum''' '''lorem ipsum''' foo '''lorem ipsum'''
