- Update debian copyright with all maintainers.
- Use newer copyright format.
* debian/manpages: add file to install manpages instead of rules.
* debian/patches: update 05-makefile.patch to add empty "all" target.
* debian/rules: simplify due to newer debhelper.
* debian/watch: update to version 4 and use fake package as there is no
longer an upstream.
