phpmyadmin 4:4.5.2-2 source package in Ubuntu

Changelog

phpmyadmin (4:4.5.2-2) unstable; urgency=medium

  * Require PHP >= 5.5 (to avoid installing on older systems which do not have
    it).
  * Symlink create_tables.sql to examples to have the file in the previous
    location as well.
  * Remove mcrypt dependency, it's not directly needed.
  * Add alternative dependencies on PHP 7.
