    Write init lock initiated by async.c

    Since the writelock is now reader based and not module based, cs writelock during card init done out off ifd_async.c. Write unlocked after full card init done .
    cd on triple only ok for one card still a problem which i could not solve yet.


  * [r9003]

    Fix http refresh in readers and cacheex.

    This fixes ticket #3457. Patch by AbrahaM.
