webif: Load external jquery files using // protocol (auto http or https).
When SSL and external jquery is used in webif the browser comlains about
usage of insecure elements in secure page. The browser is right to complain.
This fixes ticket #3712.

2) monitor client appear/disappear trough poll (a monitor client times out after 120 secs inactivity also if logout oscam waits 120 secs)
3) reader added/reappear with poll after reset as soon it is initialized
4) entitlements for physical reader and cccam proxy now written by poll (no clue about gbx code)
5) fix "readers added (double) if restart oscam with active poll" On next poll superflous readers are deleted
6) some smaller fixes
