not possible to change focus to some applications

Bug Description

Every few days, some applications loose the ability to switch to them.

On the Unity panel, these application's "triangle" (see the attached screenshot) are not white anymore - when selecting it, the application's window doesn't come up.

When the application is selected with alt-tab, their window doesn't come up.

When all other application windows are minimized, it is possible to see the affected application, however, they don't have the menu anymore (File, About etc.) nor the close/minimine/maximize indicators.

The "differently coloured" triangle refers to Firefox being on *another* workspace, not the current one.

From what I'm guessing, the issue is occuring because multiple Firefox windows are open (or probably one real and one ghost window), and one of them is getting matched properly by BAMF, while the other (or the real window) remains un-matched and that's why clicking the firefox launcher icon does nothing. And that's why the real window doesn't have any menus, title or window buttons.

Do you note a pattern with why this issue occurs? Does it occur only with firefox? Does it occur when doing only certain tasks (running a download, etc) ?

This is not the case. When I click on the workspaces button, there are no other windows on different workspaces.

Also, when I minimize all other windows on that workspace, I see Firefox eventually.

I saw this issue with other applications, like lastfm or kadu.

If it was the case, we should see more than one triangle for that application, or?

No idea. Never seen this issue with other window manager, but see quite regularly with Unity.
