1.) KDE on Gentoo already comes with scripts /etc/kde/startup/agent-startup.sh and /etc/kde/shutdown/agent-shutdown.sh. You only need to uncomment the lines for gpg-agent in those scripts.

2.) You can configure the GPGSM backend through KMail by choosing "Configure KMail" from the "Settings" menu, going to the "Crypto Backends" tab on the "Security" page, selecting "S/MIME (gpgsm)", and clicking "Configure". There should be no need to manually edit GPGSM's configuration files.

3.) Emerge kde-base/kleopatra. Then choose "Certificate Manager" from the "Tools" menu in KMail. That will launch Kleopatra, where you can import your certificates and private keys using a fairly intuitive GUI. There should be no need to invoke gpgsm from a command line.
