I have the same problem when I try to use the second ATI 4670 card in a system with two of these cards. With the first one alone it works just fine. This is 10.4 64 bit with fglrx. The message in Xorg.1.log is:
Fatal server error:
xf86OpenConsole: VT_WAITACTIVE failed: Interrupted system call
