Bug Description

While upgrading to 12.04 the installer got stuck while trying to install printer-driver-pnm2ppa. I checked the terminal for a system dialog, and found, "cp: overwrite `/usr/local/doc/vsh/initrd.img'?" I tried to enter "y", "Y", "yes", "Yes", and then "n", "no", etc. all without any effect. I searched for a hidden dialog box and found nothing. I hit enter a bunch with no effect. I waited for ~1.5 hours or so, still no change so I finally killed the dpkg process and then everything went on and finished without much trouble. I've attached the main and term dis-upgrade logs for your reference.
