dmix not enabled for USB devices plugged in at boot

Bug Description

With alsa-lib 1.0.14 1ubuntu7, it appears that dmix is not enabled by default for USB devices plugged in when the system boots. As a workaround, unplug and replug the USB sound device and restart any ALSA-enabled applications (including the ALSA mixer if affected by https://bugs.launchpad.net/ubuntu/+bug/150089). Unplugging and replugging the USB sound device also fixes sound in the Firefox Flash plugin.

This might be considered a regression of https://bugs.launchpad.net/ubuntu/+source/alsa-lib/+bug/33736.
