Bootstrap OPAC: My Lists could use formatting help

Bug Description

In the Bootstrap OPAC (3.6.1), the My Lists section of the page could use some display help as it is difficult to interpret the page. See attached screenshot.

Possibly stretch the title and description across the top with the buttons underneath it?

Perhaps the "Create a New List" section could be collapsed and display upon the click of a button? That would save scrolling for people who have already created their lists.

Better yet, have it expanded if there are no lists yet, and then collapse it if there is at least one list created?

The patch moves the buttons for each list to be underneath the title and description and displays a bottom border line for each row.

The create form is now collapsible, and expands when there are no items in the list, or a logical menu function is chosen, such as, add basket to list, or if the button is pressed.

If the add from basket options are chosen from a button or menu item, the move contents of basket to this list option is automatically set to 'yes' in the form.

This looks great. The only thing I'd add is when adding things from the basket, it's still a little confusing. In my case, it was confusing because I knew I wanted to add to an existing list and was disconcerted by having the "create new list" form open. But, it also makes sense to me that this would be a great way to start a new list.

So.
