CouchDB options list is not destroyed in new address book dialog when changing type from CouchDB to something else

Bug Description

Binary package hint: evolution

This is a GUI bug in the New Address Book dialog.
Steps I followed :
- In the main window, File menu -> New -> Address Book
- The New Address Book dialog appears, address book type is set to "CouchDB" and CouchDB specific options are displayed below
- Change the type from "CouchDB" to "On This Computer" (or anything else)
- The "Server" part of CouchDB specific options remains displayed in addition to the current specified type options
What should normally happen :
The CouchDB server options should not remain displayed.

Here is a screenshot showing the problem.
I forgot to mention that the "Server" part goes above other options after changing type.
