I can't reproduce this here at all. Can you please apply the attached patch to /lib/plymouth/themes/ubuntu-logo/ubuntu-logo.script and let me know the contents of the debug line that gets spit out at the top left of the screen?

I tried this but found nothing actually printed out, tried about a dozen
boot attempts.

I also did a s/# Debug/Debug/g on the whole file and changed the text
from white to green... still didn't see any debugging text shown during
boot. However I did see a green error message shown during shutdown.

I can't see any evidence that this file is controlling my theme -
/lib/plymouth/themes/ubuntu-logo/ubuntu-logo.script - could it be I have
some cruft theme file somewhere? Or do I need to rebuild init.d or
something?

The initramfs is supposed to be rebuilt on each package upgrade. Please check where /lib/plymouth/themes/default.plymouth points (or just run apport-collect 553708), and try running 'sudo update-initramfs -u'.

I gave up trying to debug it and wiped and reinstalled. From what Steve said it sounds like it's some bug that crops up on upgrade.
