New support ticket email doesn't include package name

Bug Description

I'm subscribed to all ubuntu support requests, however there's some applications I know nothing about and can't really help with. Some support descriptions are pretty vague and assume you know what package they're talking about.

It'd be nice for the email to include the package name to save me from clicking on the link and visiting the website.

To fix this we could move the Source package field from +sourcepackage to the form at +addticket.
