Bug Description

Sometimes, when a maximised window (particularly Nautilus) is focused, it is impossible to "un-hide" the launcher by touching the right hand side or top corner of the screen with the mouse cursor. In this case it is necessary to activate the Dash with <Super>, change to another workspace, or un-focus the maximised app.

Same here. It doesn't seem to by related to any particular application but rather to the fact it is maximized.

I don't know how to reproduce it. It happened with a fresh boot. I opened Google Chrome (which was not maximized yet), maximized it, then noticed I couldn't access the Unity menu. The Super key worked fine. I then un-maximized the window, saw the launcher appear, re-maximized it, and the problem recurred. Several minutes later it was working normally again.

I believe it happens when stacking gets broken in compiz (bug 845719) but have no way to confirm it for sure. One other thing is that when it happens alt-tab doesn't display its switcher either but it does switch the windows. I can access launchbar by pressing super key or by moving the cursor to the left side bit it is invisible. On the attached movie you can sometimes see artifacts of it but on a display I just don't see it showing up at all. This resembles a little Bug #820152 and is one of the most annoying things about unity at this point.
It is pretty easy for me to reproduce. I just open several different windows including few maximized firefox'es then I close some of them and sooner or later it happens.
