include backported pvfb patch

Bug Description

The following patch introduce the kernel parts of the PVFB infrastructure backported from upstream (xen-unstable changesets 12678,12680,12798,13018,13065)

It currently needs 3.0.4+ userspace tools (working with packages from my repo : http://cedric.gabriello.fr/ubuntu)
Backporting those bits are on my todolist
