Approving a proposed team member twice will cause an OOPS

Bug Description

Just saw OOPS-67C622, and it seems to me that two users tried to approve a membership almost at the same time, or one user double-clicked the 'Approve' button.

I tried to reproduce it locally but, aparently, double-clicking the 'Approve' button doesn't cause the OOPS.

This happens when a user has two tabs opened in the approval form.
If the user approves the membership in one of the tabs, and tries to approve in the other tab, the bug is triggered.
