Attachment is silently ignored if description is left blank

Bug Description
On the new comment/attachment form, I typed in a filename to attach and pressed enter. I didn't notice, but I expect this failed to automatically check the "include attachment" checkbox, and the result was a complete no-op since I didn't enter a comment.
I think the form should error if the user provides neither an attachment nor a comment.
