(1) We don't have Dell E5440 NoteBook at WH now; and we can't reproduce this issue by our platform.

(3) We suspect this issue may cause by BIOS and Platform feature setting conflicted with our sdhci-pci-o2micro setting.
    We are asking Dell for schematic for Our Card Reader Chip now.

By the way, do you have this notebook or not?
In order to do get more info, could you help us to get lspci log for following 3 log:
        a. Use Kernel 3.13.0-031300 and Cold boot to Linux, Then "lspci -vxxxx > log1"
      b. Use Kernel 3.16 or newer and Cold boot to Linux, Then "lspci -vxxxx > log2".
      c. Then Reboot the OS, this time the card reader will disappear. Then "lspci -vxxxx > log3".
