Are you able to reproduce it by using ntlm_auth directly on the command line, like so?:

$ sudo ntlm_auth --helper-protocol=squid-2.5-basic
MYDOMAIN\testuser mypassword

$ sudo ntlm_auth --helper-protocol=squid-2.5-ntlmssp
MYDOMAIN\testuser mypassword
