There is a field labeled "Queue:" with the default value of "/printers/" in it.

It seems the "Verify..." and "Forward" buttons are disabled unless you change this default value. Changing it to anything else enables the "Verify..." and "Forward" buttons. This seems a little bit strange behavior to me, but I was at least able to use this screen once I realized that.

My printer seems to ignore the value for "Queue" anyways. I can leave it blank or write "/crap/" in it and it sets up fine either way.
