Please attach file '/var/log/Xorg.0.log' and the output of "lspci -vvnn", "sudo ddcprobe" and "xrandr ".
Btw. after that please try the uploaded xorg.conf. I have removed all unneeded modelines, modes and so on. Maybe it helps.
