"Save Changes" button not visible when "Add a comment/attachment" is expanded

Bug Description

When you click to add a comment or attachment to a bug report, you cannot see the "Save Changes" button anymore, because the page is not high enough. So you leave the keyboard, scroll down, and then hit the button. This scrolling action could be avoided, if the button is placed near the other controls.

This could be fixed in three steps.
1. When the "Add a comment" section is expanded, focus the "Save Changes" button, then instantly focus the comment field. (Borrow this code from, or share it with, the "No, I'd like to report a new bug" section on the bug-reporting page.)
2. Halve the height of the "Comment" field. (This would also discourage unhelpful chattiness.)
3. Split "Add a comment" and "Add an attachment" into separate expandable sections, with a single "Save Changes" button outside both. (This would fix bug 53635, and also prevent people from thinking that a comment was compulsory when adding an attachment.)

Step 1 would make browsers show both the "Save Changes" button and the comment field if the window is tall enough. Steps 2 and 3 would greatly increase the probability that the window is tall enough.
