[windows] copied files are duplicated

Bug Description

Files copied over MTP are shown duplicated on Windows.

Steps to reproduce:
- Windows PC->Control Panel->Device Manager
- Nexus 4 is shown under Portable devices
- Right click on Nexus 4, choose Update Driver
- click 'Browse my Computer for driver software'
- click Let my pick...
- select MTP USB Device
Windows installs MTP USB Device driver.
Unplug and plug USB cable back in.
Nexus 4 device will appear in Windows Explorer.
- Attempt to copy a file to a directory.

Click on Nexus 4 in Windows Explorer, try to copy a file to the filesystem. A copied file appears duplicated.

Expected result: Files are shown and can be copied, deleted on the device. Only one entry is displayed for each copied file.

With Windows 8 and Windows Media Player, the results are similar. No files are shown and no action can be performed.

Fix is in silo 11; available for more testing (as far as I am concerned, after testing this behavior, it appears to resolve the issue)

I don't have Windows, but I've seen a bug which looks like this using gmtp. I might be able to get some test results.

In my experience, only SD card devices (and files/dirs on the SD card) are duplicated. The device itself only ever has one entry per item.

FWIW, I saw the dupe file issue on gmtp. It was easy to reproduce -- connect to the SD card, send a file, disconnect, reconnect. There are then two of that file instead of one.

After installing the fix for this bug, that didn't happen any more.

However, I still occasionally see the duplicate-device issue, where the SD card shows up two or more times in the device list in gmtp, and each directory entry will also show up two or more times.
