[phone-app] The word "Keypad" still shows in header

Bug Description

Krillin, build r169
Steps to reproduce:

1. make sure you have to sim cards inserted
2. launch the phone app for the first time
2. dismiss the popup dialog by tapping LATER asking you to set up a default SIM
3. The header should now say "Keypad"

Expected behaviour:
The header should state PHONE instead of KEYPAD in case no default SIM was selected yet.
This issue was partially resolved in this bug: https://bugs.launchpad.net/ubuntu-ux/+bug/1353959

It is still an issue and it would be great to change it. We normally have the name of the app in the header. Putting "Keypad" there is a weird thing to do and feels misleading.
