I think this may be a dupe of bug 411574 but since that was about a longer standing problem I thought it best to report it as a separate bug. This has just started happening for me after rebooting into 2.6.31-12-generic

Please confirm with 2.6.31-13.42-generic. If you're able to reproduce it in
the new kernel, please also add the ubuntu-audio-dev PPA.

Just booted into 2.6.31-13-generic #42-Ubuntu and still experiencing the same issue. Will install PPA and see if it still occurs.

Adding the PPA has fixed my problem. I've removed the PPA again so I'll get system upgrades which will hopefully override the PPA packages once they land.

Thankfully, this bug and bug 411574 are different, though this bug certainly exacerbates the latter.
