"Also occurs in..." page titles and headings need rewriting

Bug Description

The +choose-affected-product and +distrotask pages have the title "Bug #nnn - Request a fix", and the headings "Add affected {product, source package} to bug". And the following page has a button labelled "Indicate bug in xyz". These are inconsistent, and none of them really make sense (the first being my fault).

(This bug would be irrelevant if bug 1334 was fixed.)
