"Undefined index: name" when manually creating user

Bug Description

This is on master will full debugging turned on.

Going to "Site Administration | Users | Add user" brings up the attached error message.

I'm not too sure what that change was meant to do, and this problem will still occur with the "Group settings" field set in the admin options (see commit message). It doesn't seem like this change is ever used.
