Flavor should show root vs ephemeral in launch instance dialog

Bug Description

Currently there is no way to see the ephemeral disk size for a flavor so it's confusing to see the quota usage increase beyond the root volume size.

    Improvements to flavor details in the launch instance dialog. The
    relevant pieces of the flavor are now shown in a table above the auota
    usage and update dynamically when a flavor is selected.
