You mean I should change the changelog entry from:

esvn (0.6.11+1-3ubuntu1) gutsy; urgency=low

  * Merge from Debian unstable (0.6.11+1-3):
   - Fix FTBFS with GCC 4.3 (Closes: #417175)
   - Update XSBC-Original-Maintainer email address
   - Bump Standards-Version to 3.7.2 (no changes)

to something like:

esvn (0.6.11+1-3ubuntu1) gutsy; urgency=low

  * Merge from Debian unstable (0.6.11+1-3):

esvn (0.6.11+1-3) etc. etc.

  * Fix FTBFS with GCC 4.3 (Closes: #417175)
  * Update XSBC-Original-Maintainer email address
  * Bump Standards-Version to 3.7.2 (no changes)

were the 2nd entry is the last changelog entry from the Debian package?

You have to include the new debian changelog entry :

and then include a new changelog entry for ubuntu, listing the remaining changes from debian in the package
btw, it seems that you have discarded a change in debian/rules and debian/esvn-doc.doc-base :
* Changed doc directory to /usr/share/doc/esvn/ (Closes: LP#73913)

Because I've made the patch vs. the feisty version (0.6.11+1-2ubuntu1).

Now seems like you want the patch against 0.6.11+1-2:

You also have to put the former ubuntu changelog entries in debian/changelog, as the patch applies to the debian package

My apologies for the delay in responding. Yes, the last submitted changelog was exactly what was requested, and the the production of a debdiff between the Debian version and the proposed new Ubuntu version. I have marked this bug as Fix Released, as another has processed the same merge already.
